Programs for Seniors and People with Disabilities of all ages

The Home & Community Care programs Information, Assessment and Nursing Home Relocation or Consumer Directed Supports are designed to support elderly and people with disabilities in their efforts to remain living as independently in the community as possible.

The first step to access services is a call to our intake workers at 651-266-3613 for adults or 651-266-4500 for children. We will explore with you your wants and needs; what types of services may be available; review eligibility criteria for various options, and review the types of supports you already have.  If you are interested in applying for services, we will mail an application packet to you.

Assessment and Referral

Next, you can request a free assessment. The assessment, referred to as a Long Term Care Consultation, is done with a Social Worker or a Public Health Nurse who meets with you and anyone you wish to include. At the conclusion of the assessment, recommendations will be made for you to consider in determining your support needs The above waiver programs each have very specific eligibility criteria. If you meet the criteria, you may choose to participate in one of the waiver programs.
